We welcome you to our practice focused on the exclusive treatment of shoulder conditions. For your convenience, our office is situated in North Central San Antonio. We are committed to providing you with individual care and attention.

New patient information sheets may be downloaded and completed prior to your visit, thereby saving time on the day of your appointment. Also, Dr. Sledge will need to review any imaging studies performed elsewhere pertaining to your condition such as MRI, plain XR, ultrasound and\or CT. Please bring copies of the films or CDs along with radiologist reports to your appointment to allow a complete examination in our office.

Finally, please have accurate insurance information along with a photo ID available when you arrive at the office so we may enter your data into our system for effective treatment and efficient billing purposes.
